6. Bipolar Electrodes & Electrosurgical Accessories (SV-IFU-BPE-001 Rev. 01)

Electrosurgery Safety Standard

This specialized section provides validated clinical, electrical safety, and reprocessing protocols for SurgiValley reusable bipolar electrodes, bipolar coagulation forceps, and electrosurgical probes in compliance with IEC 60601-2-2, ISO 13485:2016, and MDR EU 2017/745.

Device Classification
Bipolar Electrosurgical Accessory

Supplied in reusable non-sterile configuration. Terminal steam sterilization mandatory prior to clinical use.

Generator & Connector
Valleylab-Style 2-Pin Interface

Compatible with standard international high-frequency electrosurgical generators (Erbe, Valleylab, ConMed, BOWA).

Insulation Dielectric
High-Grade Medical Polymer Coating

Validated for dielectric barrier integrity up to 3.0 kV peak high-frequency voltage resistance.

6.1 Clinical Preparation & Intraoperative Precautions

  • Visual & Dielectric Inspection: Prior to surgical field entry, inspect the entire insulated shaft under adequate illumination and 2.5x magnification. Verify that the medical polymer coating contains zero cracks, peeling, punctures, or thinning. Where institutional SPD high-voltage insulation testers are available, verify continuity across the active tips and connector pins.
  • Direct Visualization Mandate: Never activate the electrosurgical generator without unambiguous direct visualization of the target tissue site. In confined anatomical spaces (e.g., anorectal, transoral, or minimally invasive micro-access), ensure no inadvertent contact occurs between the active electrode tips and adjacent non-target tissue or conductive metallic retractors.
  • Lowest Effective Power: Begin generator output at the lowest clinical setting recommended by the electrosurgical generator manufacturer and increase incrementally only if required for adequate coagulation. Use short, controlled bursts rather than prolonged continuous activation.
  • Tissue Eschar Cleaning: During active surgery, remove accumulated carbonized tissue eschar from the active tips using a sterile moist gauze or approved silicone tip cleaner. Never scrape active tips or insulated shafts with scalpel blades, needles, or abrasive metal scourers, as micro-scratches destroy the non-stick surface and compromise dielectric coating.

6.2 Validated Bipolar Cleaning & Sterilization Matrix

Reprocessing Stage Required Parameter / Protocol CSSD Acceptance Threshold
1. Point-of-Use Wipe Moist saline-free wipe immediately after procedure. Transport in closed, humidified container. Zero dried blood or tissue crust on tips or connector recess.
2. Manual Enzymatic Cleaning Immerse in neutral pH enzymatic detergent (pH 7.0–8.5) at 20°C–40°C for 5–10 minutes. Clean with soft nylon bristle brush. Complete removal of organic debris; no fluid retention inside connector socket.
3. Demineralized Water Rinse Flush thoroughly with demineralized/DI water (<10 µS/cm) to eliminate detergent residues. Residual detergent concentration <0.5 µg/cm²; zero mineral spotting.
4. Moisture-Free Drying Dry tips and connector socket with filtered, oil-free medical compressed air (<2 bar). Absolute absence of moisture in connector pins (prevents electrical arcing).
5. Pre-Vacuum Steam Autoclave Steam autoclave at 134°C (273°F) for 3–5 minutes under 2.2 bar pressure; minimum 20-minute vacuum drying phase. Sterility Assurance Level (SAL) 10⁻⁶; device cooled to room temperature prior to handling.
CRITICAL ELECTROSURGICAL SAFETY MANDATE:

Never attempt field repair or re-insulation of damaged bipolar electrodes. Any electrode exhibiting hairline cracks, exposed substrate metal on the shaft, loose connector pins, or deformed active tips must be immediately quarantined and retired from clinical service.

1. Scope, Device Description & Metallurgical Specifications

This IFU governs all non-powered manual surgical instruments manufactured by SurgiValley, including dissecting scissors, hemostatic forceps, needle holders, tissue retractors, bone elevators, rongeurs, curettes, and orthopedic trauma instrumentation.

Material Class Standard Grade Hardness (Rockwell) Clinical Application Processing Precaution
Martensitic Stainless Steel ASTM F899 / AISI 420A/420B HRC 48–52 Cutting scissors, osteotomes, bone rongeurs Dry thoroughly; avoid prolonged exposure to saline or bleach.
Tungsten Carbide Inserts (TC) WC / Cobalt Brazed Matrix HRA 88–92 Gold-handled needle holders, SuperCut blades Avoid thermal shock; inspect brazing perimeter for micro-fissures.
Austenitic Stainless Steel ASTM F899 / AISI 304 / 316L HRC 25–30 (Tensile) Retractor blades, cannula shafts, suction tubes High corrosion resistance; clean internal lumens with nylon brushes.
Medical Titanium Alloy ASTM F136 / Grade 5 (Ti-6Al-4V) HRC 36 Micro-ophthalmic, cardiovascular & MRI-safe sets Non-magnetic; clean with neutral pH enzymatic detergents only.

2. Point-of-Use Handling & Containment

Immediate O.R. Pre-Cleaning

Gross soil, blood, saline, and bone debris must be wiped from instruments immediately post-procedure using sterile water-moistened sponges. Never allow biological soil to dry onto instruments.

CRITICAL WARNING: Never soak surgical instruments in physiological saline (0.9% NaCl). Chloride ions rapidly attack the passive chromium oxide layer, inducing catastrophic pitting corrosion and stress cracks.

Safe Containment & Transport

Place used instruments in puncture-proof, closed transport containers. If reprocessing is delayed beyond 60 minutes, apply a dedicated enzymatic pre-treatment foam or gel to maintain moisture until washer staging.

  • Keep ratchets, box-locks, and hinges in the open / unlocked position.
  • Segregate delicate micro-instruments and fiber-optic cables from heavy orthopedic mallets and retractor frames.

3. Decontamination, Ultrasonic Cleaning & Thermal Disinfection

Step A

Ultrasonic Bath Decontamination

Cavitation shockwaves penetrate micro-recesses, box-locks, and serrations where manual scrubbing cannot reach:

  • Frequency: 35 kHz to 45 kHz.
  • Bath Temperature: Keep below 40°C–45°C (prevents protein coagulation).
  • Detergent: Multi-tier enzymatic, neutral pH (7.0 to 8.5).
  • Immersion Time: 5 to 10 minutes in full open position.
  • Fluid Maintenance: Degas fresh solutions and change fluid daily.
Step B

Manual Lumen & Channel Flushing

Suction tubes, cannulas, and modular handles require dedicated mechanical brushing:

  • Use soft medical-grade nylon brushes matching internal lumen diameter.
  • Never use wire or steel brushes (causes abrasive iron contamination).
  • Flush lumens with pressurized water-jet or syringe until effluent runs completely clear.
  • Actuate box locks and sliding assemblies under running solution.
Step C

Automated Thermal Disinfection

Validated automated washer-disinfector cycles conforming to ISO 15883-1/-2:

  • Pre-Rinse: Cold demineralized water for 2–3 minutes.
  • Cleaning Phase: 55°C for 5–10 min with low-foaming detergent.
  • Neutralization: Acidic neutralizer (citric acid based).
  • Thermal Disinfection: 90°C–93°C (A0 >= 3000) for minimum 5 min.
  • Final Rinse: Reverse osmosis (RO) or DI water (conductivity <15 µS/cm).

4. Validated Steam Autoclave Sterilization Parameters

Moist heat steam sterilization is the gold standard for SurgiValley reusable instruments. Healthcare facilities must operate sterilizers validated to EN 285 / ANSI/AAMI ST79 standards.

Autoclave Cycle Type Sterilization Temp Chamber Pressure Minimum Exposure Hold Minimum Vacuum Drying
Fractionated Dynamic Pre-Vacuum (Recommended EU / UK) 134°C (273°F) 2.2 bar (32.0 psi) 18 Minutes (or 4 min hold + prion protocol) 20 to 30 Minutes
Pre-Vacuum High-Temp (US ANSI/AAMI ST79) 132°C (270°F) 2.0 bar (29.0 psi) 4 Minutes 20 to 30 Minutes
Gravity Displacement (Traditional Standard) 121°C (250°F) 1.1 bar (16.0 psi) 30 Minutes 30 to 45 Minutes
WET PACK SAFETY INVARIANT: Inspect every pack immediately following sterilization. Instruments must be completely bone-dry. Any residual moisture inside pouches or trays invalidates the sterile barrier. Wet packs must be investigated, repacked with fresh wraps, and re-sterilized.

5. CSSD Functional Testing, QC Benchmarks & Lubrication

Prior to packaging and autoclave loading, technicians must perform 100% individual functional and mechanical checks:

Surgical Scissors QC

Blades must open and close smoothly without catching. Test sharpness by making 3 continuous cuts through two layers of sterile surgical latex glove:

PASS: Crisp, clean slice with distal tip without pinching, chewing, or folding.

Needle Holders & Ratchets QC

Clamp a 4-0 curved suture needle in the first ratchet click. Apply gentle manual torsional twist to the needle:

PASS: Zero rotational slipping. Ratchet lock holds under lateral tap without springing open.

Tissue Forceps & Alignment

Close tips gently. Teeth (1x2 or multi-tooth DeBakey) must engage symmetrically without lateral crossover:

PASS: Perfect tooth nesting; guide pin slides into receptacle with zero binding or friction.

Medical Lubrication

Apply water-soluble, non-silicone medical instrument lubricant ("instrument milk") to box locks and ratchet joints:

PROTOCOL: Steam-permeable formulation only. Prevents friction wear and hinge fretting corrosion.

End-of-Life & Decommissioning Criteria

Instruments must be immediately removed from service if exhibiting: (1) Deep pitting or stress cracks near joints; (2) Cracked, chipped, or delaminated tungsten carbide jaw inserts; (3) Sprung or slipping ratchet mechanisms; (4) Micro-nicks in cutting blades that cannot be hand-honed; or (5) Loss of passive oxide coating leading to persistent rust stains.
